Bone Health While on Blood Thinners

What Patients Need to Know About Vitamin D, Vitamin K, and Calcium

At Now Health, many of our patients are working on two important goals at the same time:

  • Preventing stroke or clots with medications like Eliquis
  • Protecting bone strength as we age

It’s common to wonder whether supplements like vitamin D, vitamin K, or products such as AlgaeCal are safe when taking a blood thinner.

The answer depends on which medication you take and your individual health picture—but most people can support bone health safely with a thoughtful plan.

Let’s walk through it together.

Why Bone Health Matters More With Age

Bone loss accelerates after menopause and later in life. For many Alaskans, long winters, less sun exposure, and reduced outdoor activity make bone support even more important.

Strong bones are about more than density. They depend on:

  • Adequate protein
  • Resistance exercise
  • Balance and fall prevention
  • Vitamin D status
  • Magnesium and calcium balance
  • Inflammation control

Supplements are only one part of the picture.

Not All Blood Thinners Work the Same Way

This is where confusion often starts.

Warfarin (Coumadin)

Warfarin works by blocking vitamin K–dependent clotting factors.

Because of this, large changes in vitamin K intake can affect INR levels.

Patients on warfarin need a consistent intake of vitamin K and guidance before adding supplements.

Newer Blood Thinners (DOACs)

Examples include Eliquis, Xarelto, Pradaxa, and Savaysa.

These medications work differently. They directly block clotting proteins and are not affected by vitamin K intake.

That means moderate vitamin K from food or supplements usually does not interfere with these medications.

This is an important distinction.

Vitamin D With Blood Thinners

Vitamin D supports:

  • Bone mineralization
  • Muscle strength
  • Immune balance
  • Fall prevention

There is no meaningful interaction between vitamin D and Eliquis or similar medications.

At Now Health, we usually aim for a vitamin D level in a functional range that supports bone and immune health, then adjust dosing based on labs.

What About Vitamin K?

Vitamin K helps activate proteins that move calcium into bone instead of soft tissue.

In patients on Eliquis or other DOACs:

  • Typical vitamin K2 doses are considered safe
  • It does not counteract the medication
  • It may support bone metabolism

In patients on warfarin, vitamin K intake must stay consistent.

Is AlgaeCal Safe With Eliquis?

Most AlgaeCal formulas contain:

  • Calcium
  • Vitamin D
  • Vitamin K2
  • Magnesium and trace minerals

For patients on Eliquis, these ingredients are generally safe when:

  • Calcium levels are normal
  • Kidney function is stable
  • Doses are appropriate

However, calcium supplements are not always necessary. Many people can meet their needs through food, and excessive calcium may not be helpful.

This is where individualized care matters.

When We Pause or Adjust Supplements

We look more closely when someone has:

  • Kidney disease
  • High calcium levels
  • Kidney stones
  • Active cancer
  • Recurrent unexplained bleeding
  • Complex clotting disorders
  • Upcoming surgery

In these cases, we adjust the plan carefully.

The Integrative Approach We Use at Now Health

Instead of jumping straight to supplements, we usually take these steps:

Step 1 – Check Labs

Vitamin D, calcium, magnesium, kidney function, and bone density when appropriate.

Step 2 – Build a Bone-Supportive Lifestyle

Protein intake
Strength training
Balance work
Outdoor activity when possible

Even short winter walks with traction cleats help more than people realize.

Step 3 – Add Supplements Thoughtfully

Only what is needed, at doses matched to labs and symptoms.

A Note About Bleeding Risk

Vitamin D and vitamin K at typical doses do not increase bleeding with Eliquis.

Still, any patient on anticoagulation should report:

  • Easy bruising
  • Blood in stool or urine
  • Severe headaches
  • Falls

These deserve prompt evaluation.

The Big Picture

Supporting bone health while taking a blood thinner is not only possible—it is important.

The key is a steady plan that includes:

  • Movement
  • Nutrition
  • Lab-guided supplementation
  • Medication awareness

No one-size-fits-all approach works here.
